I just don't understand why people go out without a life jacket.

Because it is a personal choice, just like Drum Runner said. I wear one when I think I need to and other times I don't. Just like smoking or drinking alcohol, it's a choice. Everyone knows it's bad for you and can kill you but they still do it anyway.
